OUTLAST San Antonio is a six-hour trail endurance challenge. Runners begin a 5K at the top of every hour for six rounds. Each lap must be finished within its hour; missing the cutoff eliminates a runner. Finishing early allows the remaining minutes for rest, hydration, nutrition, gear changes, or preparation before the next start. Completing all six laps totals 18 miles.
The course features natural trail terrain near San Antonio, including elevation changes, varied footing, and sections of pavement or gravel. Walking is permitted as long as the 5K cutoff is met. The event assesses pacing and recovery alongside speed. Four laps earn a Grit Earned patch, five laps earn a Survivor patch, and six laps earn the OUTLAST medal. Runners may utilize the basecamp aid area for water and snacks, bring their own bottle for the cupless system, and establish a tent or personal aid station. The fastest male and female based on combined 5K time receive the local OUTLAST award, finishers are added to a national leaderboard, and top competitors can qualify for the Outlast Championship.
